Claim Rejection: Subscriber & Subscriber ID Not Found. Member Not Found (My Clients Plus 3.0)

 Permissions: You must have Billing Permissions with the ability to "Submit Claims to Clearinghouse" enabled.

Claim Rejection

• Subscriber and subscriber ID not found. Member not found. 

Fix Rejection

This means the Name and or Client ID was not found by this Insurance. Please verify:

1. Client's Name - Make sure what is on the Claim matches what the Insurance Company has. 
2. Client ID - Double check the ID Number on the Client's Insurance card. 
3. Payer ID - Make sure you are sending these Claims to the correct Electronic Payer ID.
4. Check Client Eligibility 

After you have checked and/or corrected these items, Resubmit the Claim.
